Requirements:
• Active CA license
• 1-year experience in a mental facility

Responsibilities:
• Complete comprehensive, best practice, evidence-based, rehabilitative assessments documentation, and update the treatment plans when each patient’s next Treatment Conference is due
• Provide skilled treatment interventions based on assessment
• Integrate rehabilitation interventions into treatment plan conferences
• Effectively communicate patients’ rehabilitation plan, objectives, and goals to treatment team members as appropriate
• Provide quality individual education in focused areas of dietary plans, positioning plans, and use of adaptive equipment. Incorporate stage of change and learning preferences
• Educate/train unit staff and nursing staff in areas of rehabilitation services
• Apply Joint Commission, CA Department of Public Health Licensing, and Centers for Medicare and Medicaid Services regulatory standards in the provision of rehabilitation services
• Participate in meetings, professional practice groups, and hospital committees as referred
• Actively assess, evaluate, and maintain professional development to improve knowledge and skills that promote continued competence
• Participate in and identifies performance improvement criteria to improve the quality of individual care
• Be proactive in the treatment of patients as required by the hospital policy
• Provide DSH-N with patient progress reports. The report shall include patient treatment provided and follow-up care required to ensure transition/continuity of care for all patients

• Ensure that all evaluations and documentation meet requirements and standards as set forth by the CA Department of Public Health Licensing (Title 22), Joint Commission, and the DSHN Treatment Plan, and are consistent with accepted professional standards of care.

• May be required to be present during surveys conducted by regulatory agencies (i.e. California Department of Health Services, Joint Commission, and Court Monitors).
